Acceptance

  1. The NPSA does not have powers to investigate individual cases or complaints about the NHS organisation that provides your care. To find out who can help you make a complaint about your specific case, or take legal action, see Make a complaint.

  2. The NPSA does not intend to keep information identifying individual patients or healthcare staff.

  3. By completing the form that follows, you are giving the NPSA your permission to process the information you provide in order to learn about patient safety.
