National Patient Safety Agency

Report an incident

Tell us about your experience of using the NHS so we can help improve patient safety. The NPSA aims to help the NHS learn from its mistakes and, in doing so, reduce the number of things that can go wrong in the future. We do not investigate individual cases or complaints, but we do listen to patients’ experiences so we can use the information to improve safety.
