The national Reporting and Learning System (RLS) collects reports of patient safety incidents from NHS organisations.
We use this data to assist NHS bodies, and frontline staff, in improving patient safety in England and Wales.

Regular feedback is given through
Patient Safety Action Team Reports. These provide an overview of the rate of reporting of incidents to
the NRLS from acute, mental
health and primary care trusts in each SHA in England.
